I have a simple form which a user should fill out and submit. The submitted data should be stored in some database. The form should have a possibility to retrieve data from database and some basic JavaScript support.

Is there a free online service which allows me to do this?

I know of Google Forms but they don't support scripting.

You should see if Google Apps Script meets your requirement.

See if you can use Ragic. It works like Google Spreadsheet / Google Form, and it supports scripting. It also have an HTTP API to retrieve your data in JSON format.

There's a free community version that you can use to try it out.

Have you checked Zoho Creator? Zoho Creator provides a form builder interface and an API for managing the data.

Another option is Cognito Forms, which is a free online service my team just launched.

It is similar to Wufoo and Zoho Creator in terms of being able to quickly build and embed forms. However, since Cognito Forms integrates directly into your site without IFrames, you can manipulate them with custom CSS and JavaScript.
