Where do I find a simple Quiz / MCQ app?

I'm looking for an online quiz creator which meets the following requirements:

  1. I need the takers' names and emails;
  2. I need to know how much each person scored on the quiz;
  3. The person should be able to see his score after the quiz;
  4. The above should be available for an unlimited number of people.

Try QuizSnack.

You can make surveys, polls or personality tests.
