Web Applications Stack Exchange is a question and answer site for power users of web applications. It's 100% free, no registration required.

I need a web form with about 5-6 fields and a file upload. When it's filled out, I'd like the results e-mailed to me, with the file attached to the e-mail. (I don't need a spreadsheet or database for the results.)

Is there a simple web application that will let me do this?

From what you have requested, this site I found using Google seems fitting; Jotform, Web Based WYSIWYG Form Builder.
