I'm looking for a open source web-based address book software that I can host on my own server. It should support home and work addresses, images and typed phone numbers and emails, look nice and be actively developed.

Do you have any recommendations?

All I've found are some ugly outdated php address books, or the usual CRM software suspects - but I'd like to have something standalone.
