I'm looking for a software (preferably open source) or a framework I can implement to have student registrations via our school website. We are a charter school and currently have an exceedingly long PDF file that parents must fill out to enroll their children.

I would like an online system (similar to the DMV) where you could start an application and fill the entire thing out digitally

Bonus points if I have the capability to output the finished application in a PDF that w

What does DMW stand for? Have a look at Google Forms. Together with Google Spreadsheets, they form a great team, and both can be easely embedded on a website. – Jacob Jan Tuinstra Feb 7 at 21:28
