Last time I tried, you can't put an image into a Google form. Example, if you want people to vote on pictures, you can't do this. People use hacks to put in their own image. Am I missing something here? How can Google miss something like this?

People use hacks to put in their own image give us an example. You could try to insert HTML tags. But that'd hardly matter if the form data is not used in any html page. – Bibhas May 10 '12 at 15:55

It is not possible to insert an image into Google forms currently.
