If you're concerned about your privacy, create a fake profile - name it something your friends would recognize or even the opposite of what your friends would recognize or random letters. In the end, creating the profile isn't a big deal and it gets you better access to your friends: pretty good payoff.

A hangout is indeed a public live stream video that is shown on Youtube, are you mistaking it for something else? Edit: What you are seeing on that page is called Hangouts on Air. You can't join such hangouts, only watch them. You can, however, join public hangouts that don't do any broadcasting.
