You can quickly revert to the older chat interface in Gmail via the dropdown menu next to your name in the Hangouts area. You'll still have the new Hangouts UI in Google+ and mobile, just not in mail.

It appears Hangouts on my Android phone works without a Google+ account, but many features such as photo sharing are disabled. Hangouts for Chrome browser extension insists on creating a Google+ account. Things could change. I do not like how Google is pushing all users to Google+.

A hangout is indeed a public live stream video that is shown on Youtube, are you mistaking it for something else? Edit: What you are seeing on that page is called Hangouts on Air. You can't join such hangouts, only watch them. You can, however, join public hangouts that don't do any broadcasting.

I have a non-technological solution to this problem: I place a Post-It note on the back of the lid, and fold over enough of the non-sticky part of it over the bezel, up over the top of the lid, and on so that it covers the lens. They default that list to 10 people. Unfortunately there is no way to remove the arbitrary list of people. If I understand your observation correctly, it also occurs when you start typing a contact's name in the search box as if you were going to chat/(hangout) with them.

If you're using Chrome, there's an extension called "Toggle Fullscreen for Hangout", which works well, and makes it truly fullscreen (by double clicking the main video). Unfortunately, I can't find an alternative for Firefox.
