But having manual process in this isn't too scalable, is it? I would have to go and add new and new users... And the whole process seems to be too complicated for the end user.

@Zoredache: WHy don't you put your comment into an answer so I can accept it because I think this is as close as possible to what I want and what Google Apps Scripts along with Sites and Docs allow me to do.

@Zoredache: That's an interesting approach. I could always add an additional reader/viewer as long as number of readers/viewers is relatively big (in hundreds at least). FYI: I can see that this number is limited at 200. After you reach this number you can use mailing lists to share with even more...

AFAIK timestamp is automatically added when you create a Form. When you'd look inside the spreadsheet columns, the first one is usually timestamp (unless you deleted it for some reason and I don't really know what happens if you do that)

"Name" only exists in this simplified example. What I'm doing is writing user stories (as per agile paradigm). They start with user role, that repeats. I may give them another column ID, but how can I ensure that users will increment the ID since IDs get mixed because of row ordering... This is also outlined in my question... And even if I'd use countif... Can you explain a bit further where to place and use this function?
