I had this same issue. Rather than go to Account, you need to look under your Profile. At the bottom of this section, you will find Connections and notifications, where you can then click to Leave Account.
I am also wanting to have a different "reply to" and "sender" email for Mailchimp.

I don't think this is a current option for a Mailchimp campaign.
Unless you are willing to code your own template, basic templates have predefined sections that cannot be duplicated.
That is why when you go to the Content tab you do not see any columns option.
What you can do though, is to add more elements by duplicating them or by using drag and drop. But always within the predefined sections of the template.
If you just unsubscribe that user, the user can re-subscribe again and receive your emails. I suggest creating a new segment called "Blacklisted" or similar and add unwanted users to that segment (not list, but segment). When you want to send a new email blast you can set to send it to all users except for the Segment name "Blacklisted".
It appears that method 3 is a valid solution if you have some programming skills.

Basically, you would need to generate a large text file with each user's bespoke html results wrapped in *|IF:EMAILemail@example.com|* <!-- user_x content --> *|END:IF|* conditional merge tags. This would get unwieldy in a hurry.
