I can answer the question I asked and describe how I put the symptoms into remission (so far), but can't explain the underlying problem. I'll post this to help others with the same problem, but won't accept my own answer in the hope that someone else has an answer for what is actually causing problem.

I built http://OpenTokRTC.com to show people multi-party video chat functionality. It eventually replaced skype and google hangouts for me and my friends. Its open sourced and free, so feel free to take it for a spin!
Unfortunately, it doesn't have all of the features you require (like screen-sharing), but it nailed everything else. Hope it helps!

What I did:
Log out from Skype for Web
Web Skype can not call phones until you install a special plugin.
The plugin download will be shown when opening web Skype if your web browser supports it. Unfortunately it does not support ChromeOS, Linux, WinRT, and only supports a few specific browsers on Windows and Mac.
After installing that plugin, a "Call phones" button appears on web Skype:

it is currently not possible to delete the conversation made by Outlook.com chat, manually, but it is deleted by the system after a while.

Just did this and I am sure it has changed. Go to the individual transaction in Activity, click on Transaction Details. From there click on View Billing Agreement. There you will find the Cancel option.
